This narrative review synthesises five decades of clinical experience with radial artery grafting in coronary artery bypass surgery, drawing on randomised controlled trials, propensity-matched observational series, and meta-analyses. The authors conclude that the radial artery demonstrates superior patency and safety outcomes compared to saphenous vein grafting and represents the optimal second-choice conduit after left internal thoracic artery grafting to the left anterior descending artery.
